I hereby declare and inform that:

  1. the matter in question was not addressed to the President of the Energy Regulatory Office with a request to settle the dispute in accordance with art. 8 sec. 1 of the Energy Law;
  2. the matter in question is not pending and has not yet been resolved by the Coordinator for negotiations to the President of ERO, another competent entity nor the court;
  3. I have familiarized myself with the Rules of Procedure, I accept its provisions and I will comply with its provisions during the ADR Proceeding and after its completion;
  4. I agree to the forwarding and receiving letters in the ADR Proceeding via email;
  5. I agree to the transmission of copies of the documents enclosed by me; including the application and my request, in paper or electronic form to the Energy Company;
  6. I agree to the processing of my personal data within the meaning of the Personal Data Protection Act of May 10th, 2018 and Regulation of the European Parliament and Council (EU) 2016/679 of April 27th, 2016 and the Telecommunications Law of July 16th, 2004 by
